Information will be used only by Town staff or authorized personnel and retained only as long as necessary. By submitting this form, you consent to the collection, use, and retention of this information.

Permission & Agreement

I, as the parent/legal guardian of the above-named participant, acknowledge that participation in the Town of Holyrood’s Camp A’hoy (“Camp”) may involve certain risks, including but not limited to outdoor activities, field trips, and other camp-related events. I hereby: Release of Liability: Agree to release and hold harmless the Town of Holyrood, its employees, volunteers, and representatives from any and all liability, claims, demands, or causes of action arising out of any injury, accident, illness, or loss that may occur during participation in the Camp, including travel to and from Camp activities. Medical Authorization: Authorize the Town of Holyrood and its representatives to seek medical treatment for my child in the event of an emergency if I cannot be reached. I understand that I am responsible for any medical costs incurred. Assumption of Risk: Acknowledge that my child may participate in activities that carry inherent risks and agree that they participate at their own risk.
